The borders of Orlais are also flexible / disputed in places (such as along the Fields of Ghislain). (I will own up to one actual mistake: I believe there should be a Duchy of Savrenne on there somewhere but I didn't spot that until it would have meant re-doing a bunch of it to fix. My head-canon is that it's because the majority elven population of the city means its not seen as having the same prestige. The exception to this is Halamshiral, which is specified in-game to be ruled by a Comte. I did vaguely use the rule that any small region with a town or a city is a Viscomte or Marquisate. So there isn't really a hard and fast rule about whether a region is labelled a Duchy, Marquisate, Viscomte or Comte.
